Human in the loop: what it means for AI agents
Human in the loop (HITL) is a design in which people review, approve or correct an AI system's work at defined points before it takes effect. In business agents it usually means the agent handles routine cases alone and routes exceptions and high-stakes decisions to a person. Where the line sits is a business decision, written down before go-live.

Levels of human involvement
| Pattern | How it works | Fits when |
|---|---|---|
| Human in the loop | A person approves before the action happens | Decisions carry money, legal or reputational risk |
| Human on the loop | The agent acts; a person monitors and can step in | High volume, low risk per item |
| Exception-only | The agent refers only what it cannot decide | Rules are clear and evals are strong |
Designing the handoff
- Give the reviewer a pre-filled decision with the evidence, not a raw file.
- Make approve, edit and reject one click each, in bulk where possible.
- Record every human correction, and feed it back into the evals.
- Move the line as evidence builds, never by default.
In practice
Native helped DKF Recruitment build four agents that post jobs, source candidates, run outreach and book interviews, with a recruiter approving every major decision from one dashboard. At TIE, underwriters receive a pre-filled decision on the endorsements the agents refer.

Frequently asked questions
What does human in the loop mean in AI?
It means a person reviews or approves an AI system's output at set points before it takes effect, rather than the system acting entirely on its own.
Does human in the loop slow AI down?
Only for the cases routed to people. Well-designed systems decide routine cases alone and send a small share, with the evidence attached, to a reviewer.
When should AI act without human review?
When the rules are clear, the decision is low risk or reversible, and evals on real past cases show the system reaches the right outcome reliably.
What is the difference between human in the loop and human on the loop?
In the loop, a person approves before the action. On the loop, the system acts and a person monitors and can intervene.
